Baylor Scott & White's Population Model Expansion at Bonton Farms: A Place-Based Quasi-Experimental Study Linking Medical and Social Services
Scope of Program
Through funding from Southern Star Medical Research Institute (SSMRI), Baylor Scott & White Health and Wellness Center at Juanita J. Craft Recreation Center (BSW HWC) is launching a program evaluation study to expand their population health model to the Bonton Farms neighborhood of South Dallas and assess the health impact of their combined models. The primary aim of this study is to assess how cardiovascular health and quality of life are impacted through the collaboration among these partners. BSW HWC programs and services are designed to improve community health through evidence-based prevention and intervention services. They include one or a combination of the following: 1) nutrition and health education classes, 2) access to fresh fruits and vegetables, and 3) physical activity sessions. In addition to cardiovascular health and quality of life, the study will assess additional health impacts related to psychosocial factors (e.g. stress, resilience), chronic disease indicators, and identify which aspects of their combined programming provide health benefits for subgroups of residents (e.g. older adults).
